ALL PE Semester 1 Students-Informed Consent Forms-Parent Reminder

Please go into your child's PowerSchool account in the parent portal to complete these 23/24 permission forms (only for Sem.1 PE students, Sem.2 will be available in Sem.2). 

You must click on the words "Consent Forms" (next to Order Forms), to see the form icon.  It is important you complete this form by October 31/23 for your child to partake in PE activities and field trips.  See more detailed instructions below:

  1. Open the Parent Portal in PowerSchool
  2. Go to Forms and Fees
  3. Important - *Go to Fees* (where you normally make payments for school fees)
  4. Click on your Student’s Name (if more than one student)
  5. Scroll down to the bottom of the page
  6. Click on the words “Consent Forms
  7. You will see a box that says PE Informed Consent Form for the appropriate grade and semester—click on it to access the form
  8. Read the form, sign and fill out the boxes indicated
  9. Once complete, go to the bottom and click on “Submit”
  10. If you missed a box, the form will indicate this.  Please go back, complete it and hit “Submit” again
  11. You are done, thank you
